When installing the Kathy theme, the categories field on
the left of the main menu did not appear. Also when turning the categories on
in the theme settings - portfolio settings - 'show portfolio category
menu', it did not appear.

get the categories menu on the left of the main page?
In this case, I would suggest checking out the tutorial in the theme documentation. In the doc, please go to "How To > Create and display your portfolio" section and you will find the step-by-step instruction there.
Note that you can find the doc right in the purchased package. I hope it helps.
